2025 Registration: 
All athletes will need the following items in order to participate in the 2025 season and turn in at the gear fittings.  If you are unable to bring printed copies to the gear fitting we will have extra copies available.  Players will need to have all items submitted in order to start practice.


* Sports Physical-
-the form is located on the website under FORMS tab.
-Sports physicals must be dated no earlier than April 2025 and can be turned in at gear fitting or up to the start of camp.
*Proof of school enrollment
-All athletes registered in the 8u, 10u and 12u age groups can use either their original birth certificate, or a current school enrolment form.  All athletes registered on the 14u teams MUST provide a copy of their current school registration.
*2025 SYF Code of Conduct
-the form is located on the website under the FORMS tab, must be signed by the legal guardian.
*2025 SYF Waiver and Release
-the form is located on the website under the FORMS tab, must be signed by the legal guardian.
*2025 Concussion/ Head injuries
-the form is located on the website under the FORMS tab, must be signed by the legal guardian.
*2025 Opioid abuse education material
- the form will be provided at sign ups by the league.
